This permit shall become null and void if the building or work described and authorized by this permit has not commenced within 180 days from the date of issuance, or if this building or
work is suspended or abandoned at any time after the work is commenced for a period of 180 days. Obtaining inspections at intervals not exceeding 180 days identifies that work has not
been suspended. I hereby certify that I have read and examined this permit and know the same to be true and correct. All provisions of laws and ordinances governing this type of work
will be complied with whether specified herein or not. The granting of a permit does not presume to give authority to violate or cancel provisions of any State or localloaw regulating
construction or the performance of the construction. If listing the owner as general contractor, I certify that I am exempt from the requirements of the State Contractor's Registration Law
under Section 3, Chapter 126, Laws of 1967. All revisions to an approved plan require DCD review and approval prior to performing work. All building and tenant occupancy permits are
required to pass final inspection prior to permit expiration and obtain a certificate of occupancy prior to use or occupancy.

CONDITIONS
City of Bremerton's approval of this application pertains only to the City's regulatory jurisdiction, and thus compliance with City
regulations does not necessarily ensure compliance with federal or state laws.
1. Prior to performing any renovation or demolition work, Puget Sound Clean Air Agency (Clean Air Agency) and
Washington Department of Labor and Industries (L&I) regulations require an asbestos survey be performed to
determine whether there are asbestos-containing materials in the work area or structure. The asbestos survey must
be conducted by and EPA-certified (AHERA) building inspector (except renovation or owner-occupied single family
residences ).
2. For more information, please contact Clean Air Agency through their web site at www.cleanair.org or by phone at
800-552-3565 or 206-689-4058. The L&I web site can be found at www.lnLwa.gov/p-ts/asbestos/asbestos.htm.
3. FIRE: Provide hood and suppression system to meet UFC, BMC and UMC standards. Trip test required at
completion. K class extinguisher required in kitchen.
4. Fire protection systems must comply with UFC, NFPA, UBC and BMC standards.
5. Premise identification required to UFC and BMC standards, 12" minimum and in contrasting color to building.
6. Provide fire extinguishers per Article 10-1 of the Uniform Fire Code.
7. Exits shall comply with Chapter 10 of the 1997 Uniform Building Code.
8. Dumpsters shall beat least 5 feet from combustible walls.

9. Type I hood required to meet UL300 standards. Suppression system shall be interfaced with fire alarm system to
provide monitoring. Hood must be tested prior to final inspection approval. K-class fire extinguisher required.

11. Install an approved reduced pressure backflow assembly on domestic water supply where water supply enters
structure. No connections are allowed upstream of the assembly.
All approved assemblies must be tested by a certified Washington State backflow assembly tester at the time of
installation, whenever serviced or moved, and annually thereafter. For more information or to schedule an
inspection, please call Bremerton Cross Connection Control, 473-5927. No meters will be set without prior
installation, inspection, and approval by Bremerton Cross Connection Control.
12. An approved grease interceptor is required at all facilities that may discharge fats, oils, and grease into the sanitary
sewer system. Contact Earl Varner at (360) 473-5927 for grease trap/interceptor requirements.
13. UBC tables 6.4 and 6.5, water service line from meter to building must be minimum 1" diameter.
14. Locate all buried utilities prior to digging. The property owner shall bear all costs for damage, repairs or necessary
relocation of underground utilities due to his activities.
15. Grease interceptor/trap is required per Append H of the UPC. Final Occupancy Approval will not be issued prior to
approval and intallation of this system.
16. No work shall be performed in the public right-of-way without acquiring a Right-of-Way Permit.

Left Turns will be prohibited on Callow. Install a "no left-turn symbol" (R3-2L) and "No Left Turn" text (R3-2P) at
both driveways. Istall a do not enter sign (R5-1) at the north entrance on Callow to prevent conflict between the
exiting vehicles and vehicles attempting to enter at that location.
Final Occupany Approval will not be given until these items are addressed. /1./

The following is a detailed outline of the information you have requested. Additional materials are
also attached for your review.
~ Current use of the site is a former 7-11 convenience store, which has since been vacated.
~ Proposed use of the site is a Chinese food take-out restaurant with limited dining area.
~ Proposed or actual parcel/structure site is 2,400 square feet (improvement only).
~ Adjacent land use includes some single-family residential development, as well as a retail car
dealership.
~ The site is located at the intersection of N. Callow Avenue (Highway 304) and First Street. The
subject site includes approximately 10 onsite parking spaces, a couple of which will be
reserved for employee use only.
~ Scope of work consists of converting the vacant former 7-11 structure into a Chinese take-out
restaurant with limited dining. This consists of the complete renovation of the gutted interior
of the building, including construction of:
· A kitchen
. An office area with storage space
. Counter space for food warmers, a soda machine, and cash register
· Dining area
~ Questions we have regarding the proposal process include:
What are the City requirements for converting a convenience store into a restaurant?
. What are the specific buildin', standards needed to be met during the renovation

BREMERTON.
Dear Mr. Wang:
The Kitsap County Health District Food Program has reviewed the submitted plans and
specifications regarding the food service operation for the Gyo-Gyo Restaurant, to be located at
104 North Callow Avenue in Bremerton, Washington. In accordance with the Rules and
Regulations of the State Board of Health for Food Service (WAC 246-215), and the Bremerton-
Kitsap County Board of Health Ordinance 1992-16, the plans are approved.
The Gyo-Gyo Restaurant will be permitted as a Type R (restaurant) food service establishment.
Prior to opening for food service or food preparation, a pre-operational inspection of the facility
must be conducted. At this time, all construction work must be completed, and food service
equipment must be installed and operating. Please contact this office at least five days in
advance of the intended opening to arrange for the pre-operational inspection.
