FOR TOUR OF WASTEWATER TREATMENT PLANT
In consideration of the City of Bremerton allowing Z Ak Jar l��_ to
participate in a tour of the Wastewater Treatment Facility at 1600 Oyster Bay Avenue South in
Bremerton, Washington on Friday, October 24, 2025, I hereby agree to accept complete
responsibility for any injuries or personal property damages I may sustain. I agree to follow
safety precautions and take full responsibility for my actions and for my physical condition. I
understand this activity may involve an element of risk and I agree to indemnify and hold the
City of Bremerton and its employees harmless from any claims, suits, and costs (including
attorney's fees) associated with my use of Bremerton facilities or the conditions at locations
during the tour, unless caused by the sole negligence of the City of Bremerton.

Youth under age 18 may not execute this agreement and the below must be signed by the
child's Parent or legal guardian.

of the participant named above and hereby consent to my minor child's participation in the tour.
I acknowledge that my child is aware that they will be entering a worksite which could contain
potential safety hazards and agree to ensure that my minor child follows safety precautions and
protocols as directed by City personnel. I take full responsibility for my minor child's actions
and for their physical condition. I also understand this activity may involve an element of risk
and expose participants to those risks. I agree to defend, indemnify, and hold the City of
Bremerton harmless from and against all claims, suits, loss, liability, or expense, including
attorney fees and costs, which may result from my minor child's participation in the tour.
