Rule 170 Rule 171 DESCRIPTION OF CHANGES INCREASE IN CONTRACT AMOUNT ($) (DECREASE) IN CONTRACT AMOUNT ($) CONTRACT TIME EXTENSION (DAYS:) X RECONCILATION OF WORKING DAYS WITH WORK CHANGE DIRECTIVES As of March 3, 2023, the City has initiated five (5) Work Change Directives (WCD’s) to the Contractor for requested changes in the project. WCD’s #1, #2, and #5 have added quantities and time to the project. WCD #1: In removing the existing CB to install MH-1002, an existing 10- inch concrete line was uncovered with continuous flow through it and no apparent upstream structure. Contractor was instructed to install a new saddle basin (CB 201) in Robin Avenue to be able to intercept flows and bypass them to set MH-1002. WCD #2: The original contract minimized paving. Only the east half of Robin Avenue was to be ground and overlaid once the storm main was installed, with patching where laterals cross over to the west side of the street. To avoid numerous crack seals and potential roadway failures in these areas, the City has chosen to pave the entire street. This will increase the following quantities: Bid Item #3 – Project Surveying – lump sum, adds additional day of survey and mobilization. Bid Item #7 – Project Temporary Traffic Control – lump sum, adds additional 5 days for traffic control Paid under Minor Change $2,150.40 $4,125.00 6 DocuSign Envelope ID: 885D001E-7AF4-4150-A016-3A41C77846A8 CONTRACT CHANGE ORDER NO: _1_ PROJECT NAME: Pine Road Basin Stormwater Improvements Project Page 2 of 3 Change Order No. 1 Rev. 09/2021 Bid Item #27 – HMA Cl. ½ In.
