LIST OF SUBCONTRACTORS
The Bidder shall provide the following information and include with this bid or submitted within 24
hours of the Bid Opening, for each Subcontractor who will perform work or labor or render service
to the CONTRACTOR in or about the construction of the work or improvement in an amount in
excess of 10 percent of the CONTRACTOR's total bid. “Rendering service” includes a
subcontractor that specially fabricates and installs a portion of the work or improvement. The
CONTRACTOR shall list only one Subcontractor for each portion of the WORK as is defined by
the CONTRACTOR in its bid. If no Subcontractor is listed, the bidder acknowledges that it does
not intend to use any subcontractor whose work on the CONTRACT will exceed 10 percent of the
bid price. Attach additional sheets if required.
