Description: Change Order # 015 for the Kitsap Lake Elementary Safe Routes to School Project
with Active Construction, Inc. This Change Order addresses the repair of the sidewalk and
sidewalk embankment at STA 109+00, 15’ RT, discovered following a heavy snowmelt and
rainfall event. This Change Order allows for additional labor and materials to repair the
sidewalk embankment damage.
Fiscal Impact: The fiscal impact of this Change Order will add $2,519.46 to overall contract
value; the additional cost impacts are accounted for from fund balance in the City’s Trans-
Capital Fund.
Action Requested: Review and signature, return to Cathy Bonsell.
**In accordance with the Certification Acceptance (CA) Agreement with WSDOT, Change
Orders must be signed by Mayor Wheeler as Approving Authority.
