B. Project Scope / Tasks:
1 . Provide weatherization and or minor home repair services to 6 homes on scattered sites throughout
the City of Bremerton
2 Accept Weatherization program applications and conduct WX program eligibility review using HUD
approved income calculation worksheet for 24 CFR part 5 definition of income
3 Facilitate Tier II site-specific Environmental Review info for eligible candidates by gathering relevant
site information and forwarding to City for review and approval before any work is done

Complete Lead Checklist on all homes and provide lead information to occupants of residences built
prior to 1978.

Inspect weatherization work, ensure lead safe practices are utilized when required and provide
copies of building permits, when required, to CDBG.
6 Follow general contractors through job progression to ensure quality assurance.
7 Ensure landlords agree to rent freeze

Track short and long term successes through the following measureable outcomes for Bremerton:
Track homeowners heating usage 12 months post on Weatherized units
Short and long term Improvement of indoor air quality and health and safety

Maintain required records and track relevant data to demonstrate CDBG program successes,
including number of unduplicated clients served, separating the counts between the City and the
County, including race and ethnicity.
10 Submit quarterly reports and billing vouchers in a timely manner
11 Submit photos (JPEG) of the program in action with your final quarterly report
C. Project Milestone Schedule
The time frame for completion of the outlined activities shall be:

Complete 6 homes for weatherization & Minor home repair 1/1/2021 12/31/2021
If any of these activities exceed the timelines by two months, a revised work schedule must be submitted to City
