The figures aren't exactly exact since there would be a lot of measuring and calculating but they're close enough for what you need. I used the blue prints from 1940 and some changes have occurred since that time such as the removal of the showers and locker rooms downstairs. Dining Hall: 1,493 ft2 Occupant load: 99 Downstairs Bar: 3,356 ft2 Occupant load: 223 Club Room (upstairs): 698 ft2 Occupant Load: 46 Lodge Hall: 2,745 ft2 Occupant Load Tables and Chairs: 183; Occupant Load Chairs: 392, Occupant Load Dancing with Perimeter Tables and Chairs: 495 Total maximum occupant load of both Club Room and Lodge Hall: 499 I suggest making an occupant load sign for the Lodge Hall with all of the occupant loads for the various configurations. I limited the maximum number of people upstairs because of the presence of two exit doors from the building. I recommend exit signs with battery backup and evaluating the emergency lighting effectiveness in the building. We discussed turning one of the lights in the Club Room to highlight the door at the north end of the room so it would be more visible in the event of a power outage. You might want to turn out the power in the main rooms to illuminate the emergency lights just to make sure the emergency lighting is effective to direct occupants to the exits. I believe some of the lights could be adjusted to better effect. I am concerned about some major items I noticed in the building. One is the cooking line in the kitchen downstairs. The deep fat fryer extends beyond the hood and isn't fully covered. If the fry station had one fryer instead of three, all of the equipment might fit under the hood with the required 6" overhang of the hood. It's a concern because the hood can't contain the smoke vapors from the grease or the flames in the event of a fire. The fire suppression company has most likely addressed the issue in the semiannual inspection report. i The second major concern is the door in the kitchen downstairs that's labeled as an exit that goes through the Boiler Room. I don't know how anyone gets to an exit, but it's definitely obstructed without a clear path of egress. I would not recommend labeling this as an exit because it gives a false sense of security. The third concern is the door to the north of the bar in the Club Room downstairs. This door is less than 30" wide and doesn't provide enough egress capacity of 32" minimum clear width as required by the code. In addition the television mounted adjacent to the door presents a vertical obstruction less than 68". The door is okay for some kind of service door, but shouldn't be identified as an exit door and you should take the exit sign down from this location. Thank you for allowing my intern and me to visit your facility. It's obvious that the building has been maintained and preserved with loving care. If we may be of further assistance, please feel free to contact me. Vaughn@dbremerton. wa. us nnie VaughnI Building Official Please be advised the bar sink downstairs and the ice machine may not be used until the drains have been properly repaired to prevent discharge to the outside of the building into the moisture sensitive soils. On September 3 and 4, 2015, several representatives from the Eagles, Public Works Department, and Department of Community Development gathered at the Washington Avenue side of your building located at 205 6th Ste to evaluate the saturated soils at the sidewalk in the vicinity of the NE corner of the building. The soils did not dry despite the dry weather, and the conclusion was water from the building was saturating the soils. While inside the building, a condensate line that drained water from the condenser at the ice machine downstairs discharged a great deal of water onto a grate on top of a drain, but most of the water was deflected by the grate onto the concrete floor. The Eagles representative was advised that this could be the source of the water that saturated the soils, although the water source was at the corner of the building and the area of saturated soils was further down the East building wall. On September 8, 2015, a dye test was used to determine if any of the plumbing fixtures in the downstairs bar were draining to the area of the saturated soils area, and the dye test confirmed the drain from the bar sink downstairs is partially or fully draining to the area of the saturated soils. The bar sink was not used for a period of time and the soils dried sufficiently to assure Public Works the sidewalk clie ould be poured. Today, the soils are saturated in the same area as before, and the sink appears to be in use. The discharge of liquids to the outside of the building will lead to undermining of the soil, undermining of the building foundation, heaving of the sidewalk in freezing temperatures, accumulation of water in the conduits for the street lighting and electrical service, and other problems. The building is situated on moisture sensitive soils and the continued discharge of the liquids into this soil will cause damage to the building's foundation and the building's walls that are supported on the foundation. Per the Uniform Plumbing Code, Section 303.1, the disposal of liquid wastes shall be through an approved drainage system, and Section 101.7 requires the plumbing and drainage system to be maintained in a sanitary and safe operating condition by the owner or owner's representative. Page 2 of 2 Because of the impact to the City's infrastructure and possible impact to the building's structural system, the bar sink downstairs may not be used at any time and must be blocked off, covered or in some way obstructed to prevent its use. In addition, the condensate drain for the ice machine must be readjusted so that it drains into drainage system and does not discharge onto the floor. A building permit is required to repair the drainage system and the wall. Please be sure to obtain a permit before performing any work on the building or the plumbing. Failure to discontinue the use of the bar sink or to repair the condensate drain line will result in code enforcement action due to the important nature of the problem and the impact to public safety. Please feel free to contact me if you have any questions or concerns. The Detector Bracket, Detector Linkage and the Fusible Link (ordered separately. The Bracket serves as support for the linkage and is attached to a rigid surface. The Linkage supports the Fusible Link and a continuous Cable under tension. At a predetermined temperature the Fusible Link will separate, relieving tension on the Cable and actuating the system. 10" Fusible Link not included with Detector This manual will refer to Series and Terminal Detectors. There is no difference in part numbers or dimensions between Series and Terminal Detectors. A Terminal Detector is the last or only detector in the network and the Cable must be "terminated" there. A Series Detector is any detector located between the Terminal Detector and the Mechanical Release Module in the Detection Network. Terminal End FUSIBLE LINKS Four temperature ratings of Fusible Links are used. They are of the electric solder type link which will melt at a predetermined temperature allowing the two halves of the link to separate. Load Umit: Max. load - 50 lbs. (22.68 kg) Min. load - 3 lbs. Crtslmuersignatt,re _ _ Hanley Pons{ntcsicm: ltQlF 1'•ty7tt' f0h 1 To: Jerry Hauth, PE Engineering Manager CC: File From: Gus Brandon Garcia Date: 9/11/2015 Re: Washington Avenue Pedestrian Boulevard Project -FOE Building Soils Condition Findings and Documentation On September 1, 2015 a defined sink hole and depression was observed on the Washington Ave project at the location of CB 10 (STA 102+55 19'9" LT). The first impression upon review of issue was that heavy rains from the previous night had created the settlement and around the new CB. The City of Bremerton initiated a consultation with the Contractor RV and Associates and the following corrective/mitigation measures were taken: • New free draining fill material approved for use on the project to fill in the depression and compacted as required in the contract documents. • The project utility testing records were reviewed and it was verified that all the installed utilities passed the required testing and had been signed off by the project CM lead (PMX) and the City of Bremerton. The following day the cement concrete sidewalk adjacent to the Eagles Building (parcel 205) was demolished and removed. It was noted at that time to native material under the walk at STA 102+55 saturated with water and observed to be unstable/unsuitable to place a new sidewalk. As part of the observation additional issues were documented in the area of the saturated soils: • The frontage of the building is assumed to be constructed of masonry blocks on top of a concrete slab with an average depth of .33' (four inches). This assumption was qualified with reps from the FOE and review of the building plans. • There are multiple voids under the slab along the building frontage, but generally relevant to the area of the North East corner of the building. No definite conclusions could be made about the size and severity of the voids under the slab. • COB field staff provided review of the existing water utility information and reviewed the existing meter location and ascertained that no current leaks have been documented by the City of Bremerton or FOE board members. • RVA staff conducted dye testing of the sink drains and plumbing of the FOE building and conclude that the drains tested were connected to the sewer. • Additional soil probing was done in the area in question by staff from PMX and COB. It was noted that the probe used bottomed out prior to hitting firm material. The current soil makeup of the existing material is assumed to have high silt and clay composition making it sensitive and expansive when saturated with water. 1 SHMNON HVILSONB INC. Construction TIMES OF SITE VISITS: From 0840 to 09300 CONSTRUCTION OBSERVATIONS NO. TOPIC AND LOCATION DESCRIPTION OF FIELD ACTIVITY, OBSERVATIONS AND RECOMMENDATIONS TO OWNER FURTHER ACTION RECOMMENDED TO OWNER 1. Review I visited the Washington Avenue project site today to evaluate the loose soils that were Avoid loose encountered below the sidewalk adjacent to the Eagles Hall building located between 5th excavation, subgrade and 6'b Streets. Loose subgrade soils were observed by Parametrix and City staff after the compaction and soils and concrete sidewalk was removed. The loose subgrade area measures approximately 5 feet disturbance of building wide by 10 feet long. I understand that the sidewalk had experienced significant cracking the loose settlement at and settlement in this area. The eastern wall of the Eagles Hall Building is extensively subgrade soils Eagles Hall, cracked at this location due to settlements that have been occurring for many years. within 4 feet the 205 6`s Eagles Hall Street I discussed the site conditions with Shannon Thompson and requested that a shallow test trench be excavated next to the loose subgrade, approximately 5 feet east of the Eagles Hall. An excavator was used to dig a 2 foot deep trench. A photograph of the test trench is attached. I used a 4400t long probe to evaluate the condition of the subgrade soils below the sidewalk alignment. Probing indicated that the subgrade soils are wet and loose to a depth of 5 feet or more beneath the sidewalk alignment. I detected the possible presence of voids in the ground beneath the sidewalk alignment. These poor soil conditions appear to extend west under the Eagles Hall and are likely the cause of cracking of the building fagade. Based on my observations and discussions with Parametrix, I recommended that disturbance of the loose subgrade next to the building be avoided. Specifically, I recommend the following: 1. Do not install a trench drain in the loose, wet subgrade area. 2. Install the new utility conduits at least 5 feet away from the East wall of the Building 3. Replace the sidewalk in this area with reinforced concrete to help span over the loose subgrade and to mitigate cracking in the event of additional subgrade settlement. 4. Avoid excavating, compacting or otherwise disturbing soils within 4 feet adjacent to the Eagles Hall in this and any other areas where loose unstable subgrades are observed. DOCUMENT REFERENCES: Plan Sheet No.(s): PP1; PP2 Spec: ATTACHMENTS: RECOMMENDED BY: ACCEPTED BY: APPROVED BY: Sketch FO September 11, 2015 On 9/3/2015 COB staff from the PW and the Building department met with the board of trustees of the Eagles to inform the board of the finding(s) to date. Access was granted by the board to inside of the building to review the existing layout. The plumbing in the street level location was reviewed and a follow up appointment was made to meet with a geotechnical engineer. The intent of that meeting is to receive some understanding of the current soils conditions and potential remedy(s) for the location that allows the project construction to move forward. 9/4/2015: Project site meeting with representatives from the following entities: COB: Jerry Hauth, Mary Ellen, Gus Garcia, Jeanie (Building Dept) PMX: Shannon Thompson Eagles board member Shannon and Wilson Geotechincal Engineer RVA: Travis (Site Foreman) The onsite meeting began at 8:30 AM. Access was granted to inside of the building and it was observed at that time that water was being discharged from a chiller unit to a floor drain and that much of the water was exiting the building to the slab and sub grade area. The geotech was onsite and 2 foot deep exploratory excavation was provided adjacent to the failure area. The location was reviewed and the following site conditions were noted: • The area in question is comprised of unsuitable/expansive soils and saturated with water at this time • The existing floor drain and chiller water appears to be the contributing source of the water in the area of failure. • A 4 foot probe bottomed out prior to reaching bearing soil As are result of the field investigation, the following field recommendations were provided by Shannon and Wilson and PMX: No excavation should commence adjacent to the building. It was the opinion of the engineer that any attempt to correct the issue would result in additional undermining of the slab and building. The installation of a curtain drain at the face of walk would have no benefit in correcting the situation. • The City of Bremerton should consider using re -enforced concrete in the area in an effort to "bridge" the poor soils. A detailed technical memorandum is expected from the consultant team with final observations and a summary of recommendations for the site 2 CITY COUNCIL Br emertoq BILL AM O District 1 E.L.
