Dear Mr. Evans:
The plans and specifications for the above project received in this office March 4, 1992 together with
information received on March 11, 1992 and telephone conversation with you on April 13, 1992 have been
reviewed and, in accordance with the provisions of WAC 246-260 (formerly WAC 248-98) are hereby
APPROVED.
As required in WAC 246-260-030 (5), upon completion of the construction and prior to use, the owner shall:
a. Submit a construction report from the engineer or architect (a copy of the form to be completed is
attached).
b. Contact the local health jurisdiction at least five working days before intended use of the facility, and
c. Obtain an operating permit through the local health jurisdiction.
This letter of approval serves as a CONSTRUCTION PERMIT under the requirements ofW AC 246-260-030
and RCW 70.90.160. WAC 246-260-030(6) provides that this permit shall be valid for a period of eighteen
months. Renewals may be granted by the department for a period of one year. It is the owner's responsibility
to apply for a renewal.
