SUMMARY: The City entered into a contract with Willis of Seattle, Inc for insurance broker services
in 2003 Since that time, Willis of Seattle has extended its agreement with the City without a rate
increase The proposed amendment extends the agreement for two years at the same rate as
initially negotiated, $25,000 per year

RECOMMENDED MOTION:
Move to approve Amendment No 5 to the Professional Services Agreement with Willis of Seattle,
Inc for insurance broker services and authorize the Mayor to finalize and execute the amendment
with substantially the same terms and conditions as presented.
