SUMMARY On August 20, 2012 the Fire Department received three quotes from the Request for
Proposal, one from Enerspect Medical Solutions, one from Zoll Medical Corporation and one from
Physio- Control, Inc On August 24, 2012 all three made companies made separate presentations
demonstrating and answering questions about their products to staff and employees The Fire
Department would propose moving forward with the purchase of three (3) Life -Pak cardiac
monitor /defibrillators from Physio- Control, Inc

FISCAL IMPACTS (Include Budgeted Amount) The department received a Fire Act Grant in the
amount of $108,042 of which the Fire Department is required to participate 10% The total amount is
approved in the Fire Department budget

RECOMMENDED MOTION -
Move to approve the purchase of three (3) Life -Pak 15 cardiac monitor /defibrillators from Physio-
Control Inc, and authorize the Mayor to finalize and execute the agreement with substantially the
same terms and conditions as presented
