This agreement commences upon signature, and termmates October 31. 2012 unless terminated earlier by the City for any reason Entry to Water Utdity lands is limited befinreen the hours of 6 00 am and 6 00 am Type of Salvage. Only wood cut from logging slash at landings as located on the attached map is inciuded in this agreement No standing trees or downed logs ma�iced as wddlife logs shall be cut or removed without the permission Qf the Forestry Manager Payment Payment for salvaged wood is /ton payable monthly to the City of Bremerton Number of Persons. Entry shall be limited to the Permittee and up to 3 additional persons Permittee shall provide in writmg the names and addresses of additional persons Entry Exceptions. No entry shall be allowed withm the Union River Surtace Supply area, wellhead facdities, or the Bremerton Police Range unless authorized by the Forestry Manager No entry shall be allowed dunng Industnal Fire Precaution Levels (IFPL) of III or higher Permittee is responsible for checkmg IFPL status No entry shail be allowed dunng inclement weather when travel on forest access roads has the potential for damage to roads or water courses Restrictions. Permittee is aliowed entry for the above express purpose only and is allowed ingress and egress to spec�ed work locations only by the most direct route Cutting and removal operations shall not cause damage to water courses, sods, roads, standmg timber or reforested areas as determmed by the Forestry Manager No smoking or open fires on Water Utdity lands dunng the closed season as defined by State of Washington regulations All other closed season regulations as defined by the State of Washington that pertam to this operation shall be observed No firearms allowed �n the Permittee's possession or in Permittee's vehicle(s) whde on Water Utdity lands Insurance. Pnor to the commencement date of this agreement, Permittee shall provide Certificates of Insurance to the City venfying the coverage under a comprehensive general liabdity ("CGL") policy providing coverage of at least $500,000 per occurrence and including Cordwood Salvage Agreement 6/28/2012 � r-== � �_ • Page 2 0 0 10 June 28, 2012 bodily m�ury and property damage The CGL shall name the City as an additional msured The CGL policy shall remam in full force and effect, and provide coverage for the full term of this agreement Insurer shall provide the City w�th 30 days wntten notice of any cancellation or change of coverage of the policy Sales Tax/Resellers Penr�it Washington State Sales tax must be collected on harvest sales unless Permittee has a valid cuRent Reseller Permit issued through the Washington State Department of Revenue, a copy of which is to be provided to the City Bremerton Business License. Permittee shall contact Bremerton's tax and license office at 360 473 5311 regarding a City of Bremerton business license Hold Harmless.
