This permit shall become null and void if the building or work described and authorized by this permit has not commenced within 180 days from the date of issuance, or if this building or
work is suspended or abandoned at any time after the work is commenced for a period of 180 days. Obtaining inspections at intervals not exceeding 180 days identifies that work has not
been suspended. I hereby certify that I have read and examined this permit and know the same to be true and correct. All provisions of laws and ordinances governing this type of work
will be complied with whether specified herein or not. The granting of a permit does not presume to give authority to violate or cancel provisions of any State or localloaw regulating
construction or the performance of the construction. If listing the owner as general contractor, I certify that I am exempt from the requirements of the State Contractor's Registration Law
under Section 3, Chapter 126, Laws of 1967. All revisions to an approved plan require DCD review and approval prior to performing work. All building and tenant occupancy permits are
required to pass final inspection prior to permit expiration and obtain a certificate of occupancy prior to use or occupancy.

1. (DCD-01) DCD: Prior to performing any renovation or demolition work, Puget Sound Clean Air Agency Lj1~
(Clean Air Agency) and Washington Department of Labor and Industries (L&I) regulations
require an asbestos survey be performed to determine whether there are asbestos-
containing materials in the work area or structure. The asbestos survey must be conducted
by and EPA-certified (AHERA) building inspector (except renovation or owner-occupied
single family residences).

Dear Sirs:
My office has notified you on two separate occasions that your building at 2641 Cherry
Court does not currently have a Certificate of Occupancy, due to the fact that a stairway
which was not shown on the approved plans now extends in the public right-of-way, and
the driveway was not installed to City standards. Your agent, Joyce Smith, met with the
City Engineering Department on site several weeks ago to determine what was needed to
allow issuance of the Certificate of Occupancy; however, an inspection performed on
January 13, 1995 showed these deficiencies have not yet been corrected.
At this time, I must inform you that use or occupancy of this structure without the
approval of the Building Official is a violation of the Uniform Building Code. If these
problems are not corrected and approved by the Engineering Department by January 31,
1995, you will be cited for occupancy violations. If you have any questions, you may
contact the Building Department at (360)478-5275.

Drs. Islam and Stanley:
As you are aware, the city has attempted unsuccessfully to resolve the right-of-way issues on your property
located at 2641 Cherry. The driveway to this site was not installed per city standards and a set of stairs which
was not shown on the original plan and never approved by the city for installation intrudes into the city's right-
of-way. Representatives from the Engineering Department have explained the problem to your property
manager, Joyce Smith, and met with her on site to discuss the situation and possible remedies; however, nothing
has been done in the 2 years since this problem surfaced. As a last resort, you were cited for allowing occupancy
without approval.
Please be advised that the city will have no choice but to continue the citation process for each and every day this
problem is not resolved if you do not take action to remedy the situation by March 18, 1996. You may contact
Ned Lever of the Engineering Department at 478-5270 to discuss your options.

The proposed Cherry A venue Medical Center is a proposed professional office building, with

construct a temporary 4,600-sfmodular office building. The building shall be served by City
of Bremerton sanitary sewer and domestic water.
Stormwater generated on the project site shall be detained in an underground corrugated
metal pipe and discharged into an existing storm line which flows south through . the Keg
parking lot. Water discharging from the structure flows in the historical manner until
reaching the Port Washington Narrows, which is located approximately 2,250-ft south of the
project site. As demonstrated in the attached calculations, the system was designed using
the City of Bremerton's "Guidelines for Storm Drainage Detention by Means of On-Site
Holding Containers".
Approximately 13, 125-sf of impervious cover is proposed for the project. The above
referenced design criteria requires O. 30-gallons of detention storage per square foot of
impervious area, resulting in 3,937 .5-gallons, or 526.4-cf of storage. The allowable release
rate is limited to O.20-cfs per impervious acre, which in this case results in a discharge of
O.06-cfs. Because the release rate is so small, the orifice diameter proposed is 1.5-inches,
which is the minimum allowed by the city. A small portion of the road approach and
parking area shall bypass the detention system. It is our feeling, however, that the bypass
runoff will be offset by the offsite runoff from the parcel to the north, which will be collected
and detained by the system.
Weare confidant that the system as designed will function adequately. There is some
concern about the downstream drainage system, but, any attempt to improve it would be
unduly costly to the proponent in both the analysis and design.
